Overview
SIST EN ISO 20737:2026 ("Health informatics - Interoperability of personal health decision support services") is an international standard developed to establish requirements for the interoperability of personal health decision support (PHDS) services. The standard specifically addresses interactions and data exchange among PHDS services, data providers, and PHDS clients within the PHDS environment. With the rapid growth of digital health applications, seamless data sharing and workflow integration have become essential for supporting accurate and timely health decisions.
This standard provides a framework for data exchange, validation, and error handling, emphasizing privacy and security considerations vital to building trust in PHDS services. SIST EN ISO 20737:2026 supports a wide range of use cases-enabling individuals, healthcare providers, and digital services to collaborate effectively to enhance personal health management.
Key Topics
Scope of Interoperability
The standard focuses on technical and organizational requirements that allow different actors-such as PHDS services, health data providers, and client systems-to work together efficiently.Data Exchange and Validation
Sets requirements for exchanging personal health data using standardized APIs, including data format validation to ensure consistency and accuracy across systems.Error Handling and Audit Logging
Specifies robust mechanisms for error management and maintaining comprehensive audit logs of all health-related data transactions.Security and Privacy
Highlights strong encryption, role-based access control, incident response, and alignment with consent-based data sharing principles to protect sensitive personal health information.Key Participants
The PHDS ecosystem includes:- PHDS services (processing and delivering recommendations)
- Data providers (e.g., EHRs, health apps, devices)
- PHDS clients (consuming and presenting decision support outputs)
Applications
SIST EN ISO 20737:2026 is foundational for anyone deploying or integrating personal health decision support tools in health informatics ecosystems. Key practical applications include:
Personalized Self-Care and Patient Empowerment
PHDS services assist individuals in making informed choices about treatments, managing chronic diseases, understanding health risks, adhering to care plans, and selecting healthcare providers-all based on secure data integration from diverse sources.Connectivity of Health Platforms
Facilitates interoperability between electronic health records (EHR), personal health records (PHR), mobile health applications, and wearable devices, ensuring a unified stream of health data for decision support.Healthcare Providers and Digital Health Vendors
Enables clinical systems and third-party digital solutions to reliably exchange information, supporting evidence-based recommendations and better patient outcomes.Regulatory Compliance and Trust
Adopts privacy and security controls aligned with global health data protection standards, supporting compliance with national and international regulations.Error and Incident Management
Provides processes for real-time notification, acknowledgement of data receipt, handling transmission errors, and enabling end-user initiated data deletion-strengthening system reliability and user trust.
Related Standards
To deliver on its interoperability promise, SIST EN ISO 20737:2026 references and aligns with several related health informatics standards, including:
- ISO 13606 (Electronic health record communication): Standardizes the structure and exchange of EHR information.
- HL7 FHIR, HL7 CDA: Widely used frameworks for structured health data exchange.
- ISO 22857 (Personal health records): Sets principles for managing and protecting personal health data.
- Relevant security and privacy standards such as those addressing encryption, access control, and consent (e.g., ISO/IEC 24791, GDPR).
Conclusion
SIST EN ISO 20737:2026 is essential for organizations and developers involved in health informatics, offering robust guidance on the interoperability of PHDS services. By leveraging its requirements, stakeholders can build secure, user-centred, and interoperable digital health solutions that deliver timely and personalized decision support-empowering better health management for individuals and supporting the broader digital transformation of healthcare.

Foreword
ISO (the International Organization for Standardization) is a worldwide federation of national standards
bodies (ISO member bodies). The work of preparing International Standards is normally carried out through
ISO technical committees. Each member body interested in a subject for which a technical committee
has been established has the right to be represented on that committee. International organizations,
governmental and non-governmental, in liaison with ISO, also take part in the work. ISO collaborates closely
with the International Electrotechnical Commission (IEC) on all matters of electrotechnical standardization.
The procedures used to develop this document and those intended for its further maintenance are described
in the ISO/IEC Directives, Part 1. In particular, the different approval criteria needed for the different types
of ISO documents should be noted. This document was drafted in accordance with the editorial rules of the

Introduction
Personal health decision support (PHDS) enables personalized decision support for self-care decisions
by processing health data provided by or on behalf of an individual. It is designed to assist individuals in
making informed decisions about their health by delivering context-specific recommendations, alerts, or
guidance derived from personal health data. PHDS is implemented as a standalone service or as part of a
health platform (for example, an electronic health record system or a health application) and can support a
wide range of use cases.
Self-care is an activity in which individuals manage their own health, including health promotion, disease
[1]
prevention and control, self-medication, and rehabilitation. The World Health Organization (WHO)
defines self-care as “the ability of individuals, families, and communities to promote health, prevent disease,
[2]
maintain health, and cope with illness and disability with or without the support of a health worker”.
WHO also says: “on average, individuals spend less than one hour a year with a health worker versus over
[2]
8 700 hours a year in self-care”. Self-care interventions are tools or strategies that can promote health,
prevent disease, manage chronic conditions, and cope with illness or disability with or without the support
[1][2][3]
of healthcare professionals. Persons choose self-care interventions for various reasons, such as
[2][4]
convenience, confidentiality, and cost. The self-care interventions are evidence-based and provided to
[5]
individuals in ways that better support self-care.
This document defines interoperability for seamless interaction among stakeholders, including PHDS
services, data providers, and PHDS clients, to support the safe and effective use of PHDS. It highlights
the importance of standardized data exchange, validation, and communication while also addressing the
essential privacy and security considerations that ensure user trust.
