Overview

ISO 21366: Traditional Chinese medicine - General requirements for smokeless moxibustion devices is an international standard developed by the International Organization for Standardization (ISO). This document establishes general requirements to ensure the performance, safety, and effectiveness of smokeless moxibustion devices. The standard addresses key factors such as smoke density, moxibustion temperature, noxious gas emissions, and test methods. It is applicable exclusively to combustion-type devices that claim to offer smokeless moxibustion and does not apply to non-combustion alternatives such as electro or infrared moxibustion devices.

Smokeless moxibustion is a modern adaptation of the traditional moxibustion therapy, designed to reduce smoke and odor while maintaining therapeutic benefits, thus improving treatment quality, safety, and user acceptance-especially regarding biocompatibility and indoor air quality.

Key Topics

  • Performance and Safety Requirements

    • Ensures that all smokeless moxibustion devices adhere to strict safety and performance parameters.
    • Covers requirements for device materials in contact with the human body and packaging durability.
  • Testing for Smoke Density and Generation

    • Detailed test methods are provided for measuring the density and generation of smoke during device operation.
    • Devices must pass both optical density and activation alarm tests to be classified as "smokeless."
  • Control of Noxious Gas Emissions

    • Specifies acceptable levels for carbon monoxide (CO), nitrogen dioxide (NO₂), volatile organic compounds (VOCs), and aldehydes, following World Health Organization (WHO) indoor air quality guidelines.
    • Mandates standardized measurement and testing routines in controlled environments.
  • Moxibustion Temperature

    • Devices must operate within safe temperature ranges, following additional requirements in ISO 18666 to prevent burns or other temperature-related hazards.
  • Packaging, Labelling, and Protection

    • Stipulates robust packaging to protect devices during transportation.
    • Defines proper labelling, including restrictions on the use of “smokeless” claims only after passing required tests.

Applications

  • Healthcare and Wellness Providers

    • Standardizes the quality and safety of smokeless moxibustion devices used in clinics, wellness centers, and home healthcare.
    • Facilitates adoption in environments where traditional moxibustion smoke is undesirable or restricted, such as hospitals, urban clinics, and residences.
  • Manufacturers and Importers

    • Assists manufacturers in designing, testing, and certifying smokeless moxibustion devices to meet international market requirements.
    • Supports product differentiation for safe, effective, and compliant devices.
  • Regulatory and Testing Agencies

    • Provides reference criteria for regulatory bodies overseeing medical devices and traditional medicine practices.
    • Enables objective third-party testing and certification using harmonized ISO methods.
  • Consumers

    • Improves user safety and experience by ensuring devices do not compromise indoor air quality or user health.

Related Standards

For comprehensive compliance and improved device quality, the following related standards are relevant:

  • ISO 18666:2021 - Traditional Chinese medicine - General requirements of moxibustion devices

Introduction
Moxibustion is one of the major traditional medical therapies. Smokeless moxibustion is an emerging form
of the moxibustion therapy. Smokeless moxibustion devices have different forms or are composed of the
materials used in conventional moxibustion without additional materials or processing for their safe use.
The standardization of smokeless moxibustion devices can enhance the efficacy and improve the quality
and safety of moxibustion treatments using smokeless moxibustion devices, especially in regard to
biocompatibility with the human body and indoor air quality.

3.1
moxibustion device
apparatus that uses moxa floss as the main combustion material and is intended for single or repeated usage
EXAMPLE Moxibustion with tube shape is a type of moxibustion device, such as a short moxa roll with a cardboard
base and a moxa tube (made of cardboard) that is single-use and developed as an alternative to direct moxibustion.
Note 1 to entry: Moxibustion device includes those accessories as defined by the manufacturers that are necessary to
enable the normal use of the moxibustion device.

3.2
smokeless moxibustion device
variant of the moxibustion device (3.1) designed to reduce the smoke and bad odour caused by combustion by
processing or changing main combustion material from moxa floss
Note 1 to entry: Carbonized moxa floss, carbonized mugwort leaves or other carbonized materials may be used as the
main combustion material.
3.3
aerosol density
smoke density
number of particulates per volume as described operationally by one of two parameters:
— m, an absorbance index, used in the testing of smoke detectors using scattered or transmitted light;
— y, a dimensionless variable, used in the testing of smoke detectors using ionization
Note 1 to entry: These parameters are not concentrations, strictly speaking, but represent values that are proportional
to the concentration and have been shown to function in lieu of a true concentration value for the purposes of these
tests.

carbon monoxide
CO
gas formed by the combination of one atom of carbon and one atom of oxygen
Note 1 to entry: In its chemical formula, C stands for carbon and O for oxygen. For the purposes of this document, the
CO level is always expressed in terms of mass fraction of CO in air.
