ISO/FDIS 17218 - Sterile acupuncture needles for single use
REDLINE ISO/FDIS 17218 - Sterile acupuncture needles for single use
ISO/FDIS 17218 is an ISO standard for sterile acupuncture needles for single use, specialized for filiform needles. It specifies requirements for the needles themselves and for the way they are sterilized, packaged, labelled, stored, and tested so users can judge whether a product is suitable for clinical use and routine quality control.
The document is written for manufacturers, test laboratories, buyers, and professional acupuncturists who need a common technical basis for product specification and acceptance. It also includes informative annexes with test methods for tip intensity and sharpness, puncture performance, and corrosion resistance.
What does ISO/FDIS 17218 specify?
ISO/FDIS 17218 specifies the requirements for sterile acupuncture needles for single use, specialized for filiform needles. The standard covers the needle configuration, materials, dimensions, performance, sterility assurance, packaging, labelling, and transit and storage.
The document is organized into Clauses 1 to 8, followed by three informative annexes. Clause 4 describes the needle configuration and types, Clause 5 gives the technical requirements, Clause 6 covers packaging, Clause 7 covers labelling, and Clause 8 covers transit and storage.
| Annex | What it covers |
|---|---|
| Annex A | Test methods for the intensity and sharpness of the tip of the needle |
| Annex B | Test methods for the puncture performance of the tip of the needle |
| Annex C | Test method for resistance to corrosion |
What are the key requirements of ISO/FDIS 17218?
Materials and biocompatibility
ISO/FDIS 17218 requires the body of the needle to have its biocompatibility assessed and documented in line with ISO 10993-1. In practice, a manufacturer can support conformity by using published data, proven clinical use, traceable materials from similar devices, or biological evaluation tests.
If the needle body material changes, or a new coating is added, the standard calls for evaluation and testing under ISO 10993-1 when there is a risk of side effects. This matters because the body enters the human body, so material changes have to be checked rather than assumed safe.
Size designation and dimensions
ISO/FDIS 17218 designates needle size by the nominal diameter of the body and the nominal length of the body, both in millimetres. That gives buyers and users a clear way to specify the needle size they need.
The standard also sets tolerances for body diameter and body length, and it requires the handle length to be at least 13 mm. It gives dimensional guidance for coiling wire handles, and for cylinder-shaped plastic and metal tube handles. This matters because correct size affects handling, insertion, and compatibility with clinical technique.
Appearance, cleanliness, and mechanical integrity
ISO/FDIS 17218 requires the needle surface to be free from particles and extraneous matter, with the body straight and without breakage or unevenness. Under x 10 magnification, the body must remain smooth and clean, and the tip must be sharp and free from defects such as scars, fringes, spikes, or hooks.
The body must also be firmly joined to the handle, with limited axial displacement under the drawing-strength test. The body must resist breakage when wrapped around a mandrel, and the standard gives a separate hardness requirement for stainless steel bodies using ISO 6507-1 or an equivalent method. In practice, these checks help confirm that the needle can be handled and inserted without failing at the root or along the shaft.
Tip intensity, sharpness, and puncture performance
ISO/FDIS 17218 treats the tip as a critical functional feature. Annex A gives a method for checking tip intensity and sharpness, including a force-based test and visual checks for hooks or bends after loading.
Annex B provides two ways to assess puncture performance - a simple qualitative membrane test and a quantitative force-displacement method. The standard uses the result to judge how easily the tip pierces material, which is important for clinical feel, consistency, and comparison between products.
Sterility assurance, packaging, and labelling
ISO/FDIS 17218 requires sterile acupuncture needles for single use to be sterilized through a validated sterilization process. The normative references point to validated processes for ethylene oxide (EO), radiation, and moist heat, which means the sterilization route has to be controlled and documented, not just applied.
The primary package must seal the needle, maintain sterility, and protect the contents from contamination and damage. The secondary package must protect one or more primary packages during handling, transit, and storage. Labelling must follow ISO 15223-1 and include the manufacturer, designated metric size, lot identification, expiry date, sterilization method, and single-use marking, with additional warnings and handling information on the secondary package.
Transit and storage
ISO/FDIS 17218 requires transport to follow the order contract and the packaged needles to be stored in a clean, well-ventilated, non-contaminated environment. The storage environment should control humidity, temperature, and direct sunlight exposure. In practice, this protects sterility, package integrity, and product performance up to the expiry date.
What terms does ISO/FDIS 17218 define?
- Body of the needle - the part inserted into the human body.
- Handle of the needle - the part not inserted into the human body, used for gripping and manipulation.
- Tip of the needle - the sharp apex at the end of the body that enters the body.
- Root of the needle - the part connecting the needle body to the handle.
- Guide tube - a slender tube that holds the needle and helps insertion.
- Sterile acupuncture needle - an acupuncture needle that has been sterilized.
- Primary package - the sealed package that directly encloses the needle and acts as the microbial barrier.
- Secondary package - the package that contains one or more primary packages for distribution and storage.
Who uses ISO/FDIS 17218?
ISO/FDIS 17218 is used by manufacturers of sterile acupuncture needles, especially those producing single-use filiform needles. It is also used by quality control staff, sterilization engineers, packaging engineers, test laboratories, and procurement teams that need to specify acceptance criteria.
Professional acupuncturists and healthcare buyers use it to understand the product format, labelling, and sterile presentation expected for clinical use. Regulatory and compliance teams can also use it as a technical basis when checking product documentation and test evidence.
What changed in ISO/FDIS 17218 from the previous edition?
ISO/FDIS 17218 cancels and replaces ISO 17218:2014 as a minor revision. The changes stated in the document are:
- Clause 2 adds additional normative references.
- Clause 5.1 has revised text.
- In 5.2.3, 5.3.1.1c), 6.1, 8.1, and 8.2, some instances of “should” were changed to “shall”.
Which standards are used with ISO/FDIS 17218?
| Standard | What it contributes |
|---|---|
| ISO 3696 | Grade 3 water for laboratory use, including the corrosion test methods |
| ISO 6507-1 | Vickers hardness test for the needle body |
| ISO 10993-1 | Biological evaluation of the needle body within a risk management process |

ISO/FDIS 17218 contains figures showing the parts of an acupuncture needle, example needle types, and example handle types. It also contains tables for dimensional tolerances, handle wire diameters, handle diameters, drawing strength, hardness, and tip pressure and puncture force limits.
The informative annexes provide practical test procedures. Annex A describes the apparatus, test conditions, and reporting for tip intensity and sharpness, Annex B gives a qualitative glove-membrane test and a quantitative puncture-force method, and Annex C gives two corrosion test methods using sodium chloride or citric acid solutions.
The document also includes package and label information requirements that users can apply directly in production and incoming inspection. For labs and auditors, the annexes show what to measure, what to record, and what to compare when checking product conformity.

iv
Introduction
This document applies to sterile acupuncture needles for single use (specialized for filiform needles) used
by professional acupuncturists for acupuncture treatment. Sterile acupuncture needles for single use are
sterilized by the manufacturer, and the healthcare professional can open the sealed package and use the
needle immediately.
In order to encourage innovation, this document does not enforce the combination of the needle diameter
and length. However, considering the clinical usage requirements, the standard provides the specifications
for the needle diameter and length.
The sharpness and puncture performance of the needle tip are of very important clinical significance.
Annex A states the guidelines and the evaluation methods for the intensity and the sharpness of the needle
tip, while Annex B provides two qualitative and quantitative evaluation methods to determine the puncture
performance of the needle tip.
The qualitative methods to evaluate the puncture performance of needle tip are described in Annex B. The
methods are simple, direct and practical. It makes them especially suitable for routine inspection and for
cross-comparison of the acupuncture needles’ clinical applications. They also play a very important role
in the enhancement of the quality of the needle tip. The methods to evaluate the puncture performance of
the needle tip can be used to further evaluate the puncture and puncture performance of the acupuncture
needle. Currently, the more appropriate method is to use the needle tip to pierce through polyurethane
material; however, this method has not yet been implemented internationally.
Considering the consistency of standards in the future, this standard provides the methods to evaluate the
puncture performance of the needle tip and ranks 5.3.5.2 as recommended. The standard does not provide
the sharpness index of the piercing through polyurethane material by the needle tip. This index will be
added to the standard when it becomes appropriate. To improve product quality, all inspection reports
should include the inspection information as well as the results of the performance evaluation.
Since every manufacturer’s design, production, and sterilization methods are different, no regulations
exist for the materials of the acupuncture needle handle. Still, the needle body and the needle handle of
acupuncture needle should have good biocompatibility.
In order to ensure product safety and efficacy, the manufacturer should perform risk analysis and enforce
risk management, and take into account relevant background data of the medical devices and clinical
practice throughout the entire duration of the product’s life cycle. ISO 14971 has provided manufacturers a
framework for the effective management of hazards associated with the use of medical devices.
In some countries, the requirements proposed here are subject to legal sanctions. Please be aware of rules
and regulations.

Introduction
This International Standarddocument applies to sterile acupuncture needles for single use (specialized for
filiform needles) used by professional acupuncturists for acupuncture treatment. Sterile acupuncture needles
for single use are sterilized by the manufacturer, and the healthcare professional can open the sealed package
and use the needle immediately.
In order to encourage innovation, this International Standarddocument does not enforce the combination of
the needle diameter and length. However, considering the clinical usage requirements, the standard provides
the specifications for the needle diameter and length.
The sharpness and puncture performance of the needle tip are of very important clinical significance. Annex
A states the guidelines and the evaluation methods for the intensity and the sharpness of the needle tip, while
Annex B provides two qualitative and quantitative evaluation methods to determine the puncture
performance of the needle tip.
The qualitative methods to evaluate the puncture performance of needle tip are described in Annex B. The
methods are simple, direct and practical. It makes them especially suitable for routine inspection and for cross-
comparison of the acupuncture needles’ clinical applications. They also play a very important role in the
enhancement of the quality of the needle tip. The methods to evaluate the puncture performance of the needle
tip can be used to further evaluate the puncture and puncture performance of the acupuncture needle.
Currently, the more appropriate method is to use the needle tip to pierce through polyurethane material;
however, this method has not yet been implemented internationally.
Considering the consistency of standards in the future, this standard provides the methods to evaluate the
puncture performance of the needle tip and ranks 5.3.5.2 as recommended. The standard does not provide the
sharpness index of the piercing through polyurethane material by the needle tip. This index will be added to
the standard when it becomes appropriate. To improve product quality, all inspection reports should include
the inspection information as well as the results of the performance evaluation.
Since every manufacturer’s design, production, and sterilization methods are different, no regulations exist
for the materials of the acupuncture needle handle. Still, the needle body and the needle handle of acupuncture
needle should have good biocompatibility.
In order to ensure product safety and efficacy, the manufacturer should perform risk analysis and enforce risk
management in addition to adhering to the requirements of local rules, and regulations, thetake into account
relevant background data of the medical devices and clinical practice throughout the entire duration of the
product’s life cycle. ISO 14971 has provided manufacturers a framework for the effective management of
hazards associated with the use of medical devices.
In some countries, the requirements proposed here are subject to legal sanctions. SuchPlease be aware of rules
and regulations should take precedence over the standards set forth in this document.
