Overview
ISO/TS 23030:2020 - Traditional Chinese medicine - Clinical document specification for prescription of traditional Chinese medicine decoction pieces - provides an entry-level implementation template for encoding prescriptions of TCM decoction pieces using HL7 CDA® Release 2.0 (ISO/HL7 27932). The technical specification focuses on the core medication-administration data for decoction-piece prescriptions to support consistent data collection, transmission, storage and exchange in electronic health records (EHRs). It does not prescribe detailed clinical document headers or unrelated body sections.
Key topics and technical requirements
- CDA-based entry templates: The specification reuses HL7 CDA Release 2.0’s substanceAdministration template as the basis for the medication-administration section.
- Defined elements and cardinality: Detailed elements (e.g., effectiveTime, routeCode, doseQuantity, consumable/manufacturedProduct) and cardinality constraints are specified, with CONF-nnnn conformance statements (CONF-0001 … CONF-0028) that implementers must follow.
- Template identifiers and OIDs: Required templateId roots are specified (for example, 2.16.840.1.113883.2.23.11.5.1.1 and 2.16.840.1.113883.2.23.11.5.1.2) to ensure consistent document recognition.
- Terminology and code systems:
- Route and site codes may use SNOMED-CT (code system OID 2.16.840.1.113883.6.96).
- Decoction method and decoction service use dedicated code system OIDs referenced in the spec (e.g., 2.16.840.1.113883.2.23.11.5.2.1 and …5.2.2).
- Structured decoction data: Templates cover decoction-piece names, dose instructions, special decoction methods, frequency (PIVL_TS), execution interval (IVL_TS), and package counts.
- Annexes: Annex A contains value sets; Annex B provides an XML example demonstrating the template in CDA XML.
Applications and who should use it
This TS is practical for:
- EHR and clinical systems vendors implementing TCM prescription modules.
- Health IT architects and integration engineers building CDA-based interoperability for TCM.
- Hospitals, TCM clinics and pharmacies exchanging electronic decoction-piece prescriptions.
- National/regional health authorities and standards committees integrating TCM prescribing into health information exchanges.
- Clinical informaticians who need consistent semantics for medication administration, CDS, reimbursement, audit and reporting.
Practical benefits include improved interoperability, reduced transcription errors, standardized data for analytics and decision support, and consistent encoding of decoction methods and services.

Introduction
® 1)
HL7 CDA Release 2.0 , which has been adopted as ISO/HL7 27932, is a document mark-up standard
that specifies the structure and semantics of clinical documents for the purpose of exchange between
healthcare providers and patients. It is necessary to establish a clinical document specification for
prescription of traditional Chinese medicine decoction pieces to support the implementation of
ISO/HL7 27932 in the context of the traditional Chinese medicine community. For implementers of HL7 ®
CDA Release 2.0, the prescription of traditional Chinese medicine decoction pieces can be provided as
a use case to maximize the use of shared semantics.
A clinical document consists of a clinical document header and a clinical document body. A clinical
document header consists of many sections, including identification and classification, participant
author and custodian. A clinical document body consists of many sections, including diagnosis,
medication administration, cost and medication plan. Usually, the core data of the prescription of
traditional Chinese medicine decoction pieces includes information about the names and amounts of
the decoction pieces, the form of processing, the dose, preparation and decoction methods, the route of
administration, frequency, the timing of consumption and the number of packages.
This document can be applied in medical institutions that need to share their prescriptions of decoction
pieces to ensure that both or multiple parties exchange documents following the specification
with consistent syntax and semantics. This document can be used to support the data collection,
transmission, storage and exchange of decoction piece prescriptions for electronic records.
In this document, the following verbal forms are used:
— “shall” indicates a requirement;
— “should” indicates a recommendation;
— “can” indicates a possibility or a capability;
— “may” indicates a permission.

3.1
cardinality
number of times that a data element can repeat within an individual occurrence or object view
Note 1 to entry: In the XML documents specified by this document, the cardinality of an element defines the
number of times the element can repeatedly appear at a specified position, which is used to specify the number
of allowed values for data attributes expressed by the name of the element. It is denoted by ‘m.n’: ‘m’ means the
minimum number of occurrences and ‘n’ means the maximum allowed number of times.

and XML schema document (XSD). For XML elements, cardinality, terminology and data type in CDA XSD, no
specific explanation is given in this document
